Medical Case Management

Care Matters coordinates and manages personalized care-plans for injuries, illnesses or other health concerns, and collaborates with healthcare and legal professionals on behalf of the person in need. We focus on wellness of the whole person to further recovery, and regain a healthy and active lifestyle.


Social Work Support

Care Matters provides counseling, resources, psychotherapy, and personal support to help a distressed person overcome issues that may interfere with their quality of life. We offer a safe place where a person can confide in someone who cares about their needs and who can help minimize or resolve their concerns.
