About Care Matters

Care Matters is a devoted advocate for individuals and families who are impacted by traumatic injuries, chronic conditions, disabilities, or everyday stressors. We provide meticulous care management from a holistic and person-centered approach to ensure the overall progression of our clients’ recovery and to minimize or remove barriers that inhibit their quality of life.

Company Overview

Care Matters, LLC was established in 2006; its founder and current director, Wendy Hooker, has provided Medical Case Management services for over 17 years and holds many professional credentials including Licensed Master Social Worker, Certified Brain Injury Specialist, and Geriatric Care Manager to name a few.

Care Matters began as a home-based business which today has two offices – one in Troy and Southfield – and a staff that has more than 50 years of combined experience in care management. The Care Matters team consists of specialized professionals including a Care Coordinator, Support Specialist, RN, CNA, Certified Dementia Practitioner, Paralegal, and Accounts Manager.
